What To Bring
Here is a list of things you should bring with you to the hospital.
Admission:
- Possibly the referral slip from your general practitioner/HNO-doctor.
- Your health insurance card
- If applicable, your health insurance company will issue a declaration of assumption of costs (only valid for 14 days).
Medical reports for the doctor:
- Brief letter from the family doctor/HNO-doctor with preliminary findings
- List and time table of all current medications
- All x-rays, CT or MRT images for your diagnosis
If available:
- X-ray of lung not older than 3 months.
- ECG not older than 1 month, pacemaker ID card if necessary.
- Current laboratory results not older than one week.
- Histology findings from biopsies taken or previous operations
- Vaccination card
- Marcumar/Falithrom pass
- Allergy Passport
- X-ray passport
- Nursing transition sheet, if you live in a nursing home or are cared for by an outpatient nursing service.
Laundry and toiletries:
- Towels, washcloths
- Soap, shower gel
- Face and hand lotion
- Toothbrush, toothpaste, mug, mouthwash
- Dental prosthesis cup and cleaning agent
- Hand mirror
- Shampoo, comb or hairbrush
- Nail Scissors/file
Individual care products (shavers etc.)
Clothes:
- Pyjamas, nightgown
- Dressing gown or comfortable house clothes
- Slippers, sturdy shoes, shoehorn if necessary
- Stockings
- Underwear
- Clothing for discharge
Other:
- Glasses
- If available:
- Hearing aid
- Walking stick, wheelchair etc.
- Adapted support stockings
- Books Magazines etc.
- If required writing utensils
- Small denomination of money for telephone or mobile phone
